What are some common diagnoses you work with?

Our most common diagnoses include Autism, ADHD, Cerebral Palsy, and TBI. However we can work with anyone who presents with functional challenges in cognition, vision, motor, or psychosocial skills.

Who is appropriate for referral?

We request that any driving referrals are close to the ages required to receive a learner’s permit or older. We also request that any clients who are referred are seizure free for at least 3 months and have corrected visual acuity to at least 20/40.

Finally, any individual you refer for a driving evaluation should be someone that you would be willing to provide medical clearance to for driving.

Please note on your referral if the referral is for any non-driving needs (i.e. community mobility training or passenger evaluations).

How can I refer my patients?

Please fax or email (if you can send it encrypted) a referral to En Route to Independence with the client’s name, DOB, diagnosis, medication they are taking, and relevant aspects of their medical chart (i.e. visit in which driving or driver rehabilitation was discussed).
